Rivian offers three PPO medical plans from Anthem:

Anthem PPO 1000

If you prefer a lower deductible, the Anthem PPO 1000 may be the best fit for you. It allows you to select your health care providers, preferred doctor, labs, and most convenient pharmacy. However, it does come with higher costs per paycheck. Keep in mind that costs for care and services will vary, and will be lower and more predictable when you choose in-network providers. Highlights include:

  • The in-network deductible is $1,000 for employee-only coverage and $3,000 for employee-plus-dependents coverage.
  • For in-network primary care visits (including virtual visits), you pay a $20 copay per visit. Specialist visits are $40 per visit. Other services generally require coinsurance.
  • For most other in-network services, you pay 20% coinsurance after meeting your deductible. Emergency room visits require a $250 copay per visit (deductible does not apply).
  • The out-of-network deductible is $2,000 for employee-only coverage and $6,000 for employee-plus-dependents coverage.
  • The in-network out-of-pocket maximum is $3,500 for employee-only coverage and $7,500 for employee-plus-dependents coverage. Out-of-network limits are higher.

Anthem PPO HSA 1700

If you’re looking for a lower monthly premium and want to pair your medical plan with a Health Savings Account (HSA), the Anthem PPO HSA 1700 may be a good fit. This plan offers the flexibility to choose your health care providers and includes a higher deductible that can be paid using HSA funds. You can also save HSA dollars for future health care expenses—including in retirement. When you enroll in a PPO HSA plan, you can contribute pretax dollars to an HSA to pay for eligible health care expenses, and Rivian contributes too. Highlights include:

  • The in-network deductible is $1,700 for employee-only coverage and $3,400 for employee-plus-dependents coverage.
  • After meeting the deductible, most in-network services are covered at 20% coinsurance. Preventive care is covered at no cost when you use in-network providers.
  • The out-of-network deductible is $3,200 for employee-only coverage and $6,400 for employee-plus-dependents coverage.
  • The in-network out-of-pocket maximum is $2,500 for employee-only coverage and $5,000 for employee-plus-dependents coverage. Out-of-network limits are higher.
  • To be eligible for an HSA, you must enroll in the Anthem PPO HSA 1700 medical plan. Existing HSAs remain with HealthEquity, and funds can be used for eligible expenses at any time.

Anthem PPO HRA 3500

If you’re looking for lower per-paycheck costs and want employer-funded help with out-of-pocket expenses, the Anthem PPO HRA 3500 may be a good fit. This plan includes a Health Reimbursement Account (HRA)—instead of a Health Savings Account (HSA)—that Rivian funds to help offset your deductible and other eligible health care costs. You can choose your health care providers, and costs are generally lower when you use in-network providers. Highlights include:

  • The in-network deductible is $3,500 for employee-only coverage and $7,000 for employee-plus-dependents coverage.
  • After meeting the deductible, most in-network services are covered at 20% coinsurance. Preventive care is covered at no cost when you use in-network providers.
  • The out-of-network deductible is $7,000 for employee-only coverage and $14,000 for employee-plus-dependents coverage.
  • The in-network out-of-pocket maximum is $4,500 for employee-only coverage and $9,000 for employee-plus-dependents coverage. Out-of-network limits are higher.
  • Rivian contributes to your HRA at the start of the plan year—$750 for employee-only coverage and $1,800 for employee-plus-dependents coverage—to help pay for eligible health care expenses. HRA funds are available at the start of each plan year but do not roll over year over year and cannot be taken with you if you leave Rivian.

  • Preventative care recommendations that can be covered with no co-payment and that do not apply to the calendar year deductible for In-network providers.

New! Anthem’s Virtual Assistant

We’re thrilled to introduce Anthem’s new Virtual Assistant, designed to enhance the digital healthcare experience by offering conversational and personalized insights, providing answers on the topics that matter most to your health and well-being.

The streamlined digital experience eliminates the need for users to navigate to different feature areas to get information. For example, a search for knee surgery offers comprehensive results, including nearby orthopedic surgeons, related benefits, questions to ask in preparation for surgery, and more. 

Members will be able to access the new Virtual Assistant on anthem.com and the Sydney Health app. When you log in to your Anthem account, you’ll notice the new Virtual Assistant tile on both desktop and mobile platforms. A simple click lets you start interacting and exploring its features.
